Depressie: risicofactoren en beschermende factoren

Depressie bij kinderen en jongeren is een ernstige mentale stoornis die vaak gepaard gaat met een hoge kans op recidive. De hoge prevalentiecijfers en de negatieve psychosociale gevolgen benadrukken het belang van wetenschappelijk onderzoek naar vroege depressiesignalen en onderliggende mechanismen van de ontwikkeling en het verloop van depressie bij kinderen en jongeren. Bovendien bevorderen resultaten van dergelijk onderzoek de diagnostiek en behandelingsmogelijkheden voor depressie en de ontwikkeling van evidence-based preventieprogramma’s voor kwetsbare kinderen en jongeren. In dit hoofdstuk worden eerst en vooral het klinisch beeld, de diagnostische criteria volgens DSM-5 en de ontwikkeling en prognose van een majeure depressieve stoornis bij kinderen en jongeren besproken. Vervolgens wordt ook aandacht besteed aan risico- en beschermende factoren en potentiële onderliggende mechanismen geassocieerd met de ontwikkeling van een depressieve stoornis. Er wordt hierbij een eerste aanzet gegeven om depressie te verklaren vanuit een transactioneel model waarin een state of the art wordt gebracht omtrent alle wetenschappelijk ondersteunde belangrijke factoren in de ontwikkeling van depressie die vaak dynamisch op elkaar inwerken. Tot slot wordt ook een update gegeven van de implicaties voor de diagnostiek en behandeling van depressie bij kinderen en jongeren en worden er belangrijke richtlijnen voor toekomstig onderzoek belicht.
