Abstract
Uit voorgaande hoofdstukken is duidelijk geworden dat een hartfalenprogramma een belangrijke rol kan spelen bij het verbeteren van zorg aan patiënten met hartfalen. In de loop van de jaren zijn in Nederland diverse hartfalenprogramma’s opgezet. In 2000 werd in Nederland in 46 ziekenhuizen en vanuit veertien thuiszorgorganisaties een hartfalenprogramma georganiseerd, meestal in de vorm van een hartfalenpoli.1 Inmiddels is dit aantal flink opgelopen.
