Zorg en nazorg voor orale implantaten

Abstract

De afgelopen dertig jaar hebben overkappingsprothesen op implantaten, vaste bruggen op implantaten, solitaire kronen op implantaten en gelaatsprothesen op implantaten zich ontwikkeld tot betrouwbare behandelmogelijkheden. Internationaal wordt zelfs gesteld dat bij protheseklachten in de mandibula een overkappingsprothese op twee implantaten de eerst aangewezen behandeling is; met andere woorden, de conventionele volledige gebitsprothese in de mandibula heeft niet langer de voorkeur als behandeling (Feine et al., 2002). Dat het plaatsen van implantaten, ook met het oog op de kosten-batenratio, een goede behandelmodaliteit is, is in 2005 al voor implantaatgedragen kronen aangetoond in het onderzoek van Brägger et al. (2005). Zij lieten zien dat kronen op implantaten in dit opzicht de voorkeur genieten boven conventionele driedelige bruggen (controle 1–4 jaar). Vooral in klinische situaties waarin sprake is van niet of minimaal gerestaureerde buurelementen en waarbij er voldoende botvolume is voor het direct plaatsen van implantaten moet volgens deze auteurs in eerste instantie aan een kroon op implantaten worden gedacht.
