Abstract
Een man van 64 jaar met al langer bestaande hypertensie bezoekt uw spreekuur. De medische voorgeschiedenis vermeldt geen relevante zaken. U controleert hem al enige tijd sinds er bij hem hoge bloeddruk was gemeten tijdens een bedrijfskeuring. U gaf algemene adviezen en drong er bij de man op aan dat hij het roken (tien à vijftien sigaretten per dag) zou opgeven. U meet nu een bloeddruk van 171/85 mmHg. U treedt weer in discussie met de patiënt. U wilt nu toch echt starten met medicamenteuze therapie. Uw patiënt heeft echter informatie via internet verzameld en stelt dat de normale diastolische bloeddruk medicamenteuze therapie niet rechtvaardigt. Bovendien, zo zegt hij, is hij gestopt met het toevoegen van zout aan het dieet en tennist hij sinds vier maanden tweemaal per week. Als alternatief stelt u nu voor om 24-uursbloeddrukmeting toe te passen. U heeft namelijk recentelijk iets gelezen over de mogelijk toegevoegde waarde van een 24-uursbloeddrukmeting. Uw patiënt stemt ermee in.
