Wat is de richtlijn voor gebruik van inhalatiemedicatie bij zwangeren met astma? Zijn inhalatiecorticosteroïden wel of niet geschikt?

Ongeveer 3-12% van de zwangere vrouwen over de gehele wereld heeft astma (1). De prevalentie zal de komende jaren toenemen, mede door de toegenomen aandacht voor de diagnostische criteria ervan (2). Verscheidene studies laten zien dat zwangere vrouwen met astma een verhoogd risico hebben op perinatale mortaliteit, (pre)eclampsie, vroeggeboorte, hypertensie, vaginaal bloedverlies en dysmaturiteit (3, 4). Twee factoren spelen hierbij een grote, weliswaar te beïnvloeden, rol: 1) de astma medicatie zelf en 2) hypoxie ten gevolge van slecht gereguleerd astma. Slecht gereguleerd astma kan leiden tot maternale hypoxie, verminderde foetale oxygenatie van het bloed en abnormale groei en foetale ontwikkeling (5, 6)
