Riassunto
La ventilazione meccanica cronica nasce contemporaneamente alla moderna ventilazione meccanica acuta. Già durante l’epidemia di poliomielite degli anni ’50 risultò chiaro che una percentuale di pazienti avrebbe necessitato di un supporto ventilatorio cronico, sia totale che parziale. Inizialmente l’unica modalità di ventilazione disponibile per un impiego cronico era quella a pressione negativa intermittente (NPV) tramite polmone d’acciaio. Si andò sviluppando un modello assistenziale basato su sedute notturne di terapia ventilatoria in ambiente ospedaliero nei casi di pazienti con residua capacità respiratoria o su ospedalizzazione cronica in caso di pazienti senza autonomia residua. Risulta chiaro che la terapia ventilatoria cronica, domiciliare o meno, era inizialmente limitata ai soggetti che avevano sperimentato un episodio di insufficienza respiratoria acuta e che, sopravvissuti, erano divenuti per lo più ventilatori-dipendenti. Il successivo sviluppo scientifico e tecnologico ha reso possibile un trattamento cronico domiciliare su un largo numero di pazienti grazie all’avvento di ventilatori a pressione negativa di ridotte dimensioni (“poncho”) e alla comparsa dei ventilatori a pressione positiva intermittente, inizialmente per via tracheostomica. La svolta epocale in questo settore si ebbe con l’introduzione della terapia ventilatoria non invasiva a pressione intermittente, grazie alla disponibilità di apparecchiature di dimensioni molto contenute.
