La carcinogenesi è un processo multifasico e multifattoriale che si sviluppa attraverso una sequenza di eventi biologici intermedi tra l’esposizione ad uno o più carcinogeni e l’insorgenza di una neoplasia invasiva. Nei primissimi stadi, l’innesco della carcinogenesi intestinale può essere evidente nella mucosa priva di alterazioni istologiche anche in completa assenza di lesioni neoplastiche endoscopicamente rilevabili. Si tratta di alterazioni cellulari o molecolari associate alla iniziazione/promozione neoplastica, conseguenti all’interazione tra fattori ambientali o microambientali ed epitelio intestinale, che possono essere evidenziate e, in alcuni casi, anche misurate in aree molto estese della mucosa [1].
