Lo scompenso cardiaco rappresenta oggi una delle principali cause di invalidità e di morte nei paesi occidentalizzati e costituisce per la sua gravità, per il costo degli interventi medici in generale e per la frequenza di ospedalizzazioni in particolare, una delle principali voci della spesa sanitaria. Nei paesi occidentalizzati lo scompenso cardiaco giustifica il 5% dei costi totali sostenuti per l’ospedalizzazione e’ 1–2% della spesa sanitaria complessiva totale [1]. L’invecchiamento progressivo della popolazione e la sempre maggior efficacia delle terapie per l’infarto del miocardio determineranno, negli anni a venire, una sempre maggior crescita dei soggetti con scompenso cardiaco cronico. Ciò rende necessario lo studio di nuovi modelli assistenziali per il paziente scompensato, la cui cura implica frequentemente il ricorso alla polifarmacoterapia e, quindi la necessità di un assiduo monitoraggio delle condizioni cliniche nel lungo periodo. Inoltre, la condizione psicologica del paziente può influenzare il decorso della patologia cardiaca e la compliance terapeutica. Parallelamente, sono di grande importanza significative modificazioni dello stile di vita del paziente che implicano una fase di addestramento lunga e laboriosa con un costante coinvolgimento dei familiari.
