End Results of Surgical Treatment: German Experience

Abstract

The cure rate for gastric carcinoma in Germany continues to be poor. Maximal 5-year survival rates, as a valid indicator of curability, are 10% [1], 11% [2], 13% [3], 16% [4], 17% [5], 22% [6], and 24% [7] and, in many cases, even worse. Japanese surgeons however, report survival rates at 5 years of 45% [8], 46% [9], 50% [10], and 62% [11]. It appears that the survival differences are due mainly to a greater frequency in diagnoses of early gastric cancer in Japan [12–14] and meticulous histopathologic evaluation of the surgical specimens [12], resulting in more accurate surgical and pathologic staging [6, 15–18], and the presumed benefit of systematic lymphadenectomy [11–14].
