Summary
We compared neurological symptoms with CT findings and SPECT findings in 38 patients with chronic subdural hematomas (CSH).
All patients had reduction in cerebral blood flow(CBF), especially in the deep cerebral areas of the basal ganglia and thalamus ipsilateral to the hematoma. Depressed CBF in deep cerebral areas was thought to be the common pathogenesis of CSH.
Cerebral diaschisis was closely related with severe dementia or disturbance of consciousness. Crossed cerebellar diaschisis was found in cases with marked midline shift on CT scan or severe hemiplegia.
Thus, diaschisis was closely related to the development of symptom in CSH. The SPECT image demonstrated that persistence of postoperative CD indicated a high possibility of recurrence.
We reported the usefulness of SPECT and the significance of diaschisis on CSH.
