Laparoscopic Retroperitoneal Lymphadenectomy for Stage I Testicular Cancer

Summary

In 1992 Hulbert and Fraley performed the first unilateral laparoscopic lymph node dissection for nonseminomatous germ cell testicular tumor; since then this operation has been carried out by several other authors. The efficacy and safety of the procedure are well established, but its role in the management of testicular tumor is still controversial. Current experience of over 100 cases suggests that unilateral laparoscopic retroperitoneal lymph node dissection is the right compromise between surveillance and open surgery in the management of clinical stage I nonseminomatous germ-cell testicular tumor. Therefore, because of the length of the learning curve, the laparoscopic route should be restricted to centers regularly attended by patients with testicular tumors. In this chapter we describe indications, technique, and results of our experience and of worldwide experiences.
