Zusammenfassung

Die Behandlung von osteochondralen Knorpeldefekten gewichtstragender Gelenke stellt ein alltägliches orthopädisches Problem dar. Fokale chondrale und osteochondrale Defekte lasttragender Gelenkoberflächen sind häufige Ursachen für Beschwerden für den Patienten (Schmerz, Schwellung, Gelenksperre, Instabilität etc.) und können zu vorzeitigen degenerativen Veränderungen führen. Es existieren einige chirurgische Techniken zur Ober flächenrekonstruktion dieser Defekte, die bezüglich des klinischen Outcomes kontrovers diskutiert werden. Traditionelle Techniken, wie Debridement, subchondrale Penetration und Abrasionsarthroplastik, haben nur eine begrenzte Möglichkeit der Oberflächenrekonstruktion wegen der Minderwertigkeit und schlechteren biomechanischen Eigenschaften des reparativen Faserknorpels [1]. In den vergangenen 20 Jahren haben zahlreiche Autoren neue Techniken zur Beschaffung von hyalinem oder hyalinartigem Knorpel für die Reparatur dieser Defekte entwickelt. Diese führten jüngst Oberflächenersatzstoffe als Alternative inklusive periostaler und perichondraler Grafts, gemorserter autologe osteochondraler Mixturen, Biomaterialien, autologer Chondrozytentransplantation, osteochondraler Allografts und autologer osteochondraler Transplantationen [2-18] ein. Die meisten dieser Techniken sind auf dem Boden experimenteller Daten entstanden, aber lediglich die autologe Chondrozytentransplantation und autologe osteochondrale Transplantationen sind ausführlich klinisch erprobt.
