Zusammenfassung
Die klassische Indikation für Heparinkonserven wird in der Austauschtransfusion, besonders im pädiatrischen Krankengut, gesehen. Die Vorteile der Heparinkonserve liegen in der fehlenden metabolischen Belastung des Empfängers mit Citrat, Natrium und Glucose, sowie nahezu normalen Thrombozytenzahlen, so daß bei großen Austauschvolumina mit Heparinblut keine Auswasch- (Verdünnungs) Thrombozytopenie auftritt. Die Praktikabilität der Heparinkonserve wird von manchen Anwendern hervorgehoben, so muß im Unterschied zu Citrat-antikoagulierten Blutkonserven keine Calcium-Substitution erfolgen. Von allen Nachteilen der Heparin-Konserve ist die Blutungsgefährdung des Patienten am bedrohlichsten. So wird in den Standards der AABB von Heparinblut abgeraten: „Use of heparinized blood is no longer recommended due to the risk of bleeding associated with its use“ [2].
