Zusammenfassung
Die Therapiebereitschaft Schizophrener wird sowohl durch mystische Vorstellungen über die Ursachen von Geisteskrankheiten wie auch durch Hypothesen über ihre Sozio- oder Psychogenese, vor allem aber durch die Verbreitung übertriebener Ängste vor der Pharmakotherapie herabgesetzt. Das hat zur Folge, daß die Patienten aufgrund unzureichender Behandlung häufiger produktiv-psychotische Symptome entwickeln oder zu einem Alkohol- oder Drogenmißbrauch verleitet werden, der die Symptomatik modifiziert. Des weiteren kann die Symptombildung auch durch diagnostische Klassifizierungen, die von den Medien der Bevölkerung vorgestellt werden, eine besondere Ausgestaltung erfahren. Schizophrene sind jedoch durchaus zur Krankheitseinsicht fähig, die ihnen die Entwicklung adäquater Bewältigungsstrategien ermöglicht. Dies kann dadurch gefördert werden, daß man ihnen hilft, sich nicht durch gesellschaftliche Vorurteile beeinflussen zu lassen, welche die Einstellung zur Behandlung und die Symptombildung in eine ungünstige Richtung lenken.

Effects of social prejudices on schizophrenics’ therapeutic compliance and development of symptoms. The readiness of schizophrenics to accept appropriate treatment may be diminished by mystical notions about the origins of mental disorders as well as by hypotheses about their pure socio-or psychogenesis and especially by the propagation of exaggerated fears concerning pharmacotherapy. This increases the number of not or not adequately treated patients and as a consequence the frequency of cases with productive psychotic symptoms. The refusal of pharmacotherapy also leads some of them to treat themselves with alcohol or drugs, which modifies their symptomatology. The symptoms may also be shaped through the presentation of patients attributed to certain new diagnostic categories which the mass-media consider as specially interesting for presentation to the public. Schizophrenics are, however, in contradiction to wide spread assumptions capable of developing illness insight in their disease which can enable them to elaborate adequate coping-strategies. This may be enhanced if they are helped to escape the influence of social prejudices which unfavorably modify their attitude towards treatment and their symptom formation.
