Zusammenfassung
Eine Gruppe von 20 wegen Unzurechnungsfähigkeit exkulpierten Straftätern mit der Diagnose Schizophrenie (ICD-9, DSM-III-R) wurde zusammen mit einer Vergleichsgruppe von 20 nicht-delinquenten, nach Diagnose, Alter zum Zeitpunkt der Ersthospitalisierung, Anzahl und Gesamtdauer der Spitalsaufenthalte gematchten Patienten im Hinblick auf das Vorliegen von antisozialen Persönlichkeitsstörungen und Substanzmißbrauch untersucht. Abgesehen von der Überrepräsentation von psychopathologischen- und Verlaufsmerkmalen wie systemisierte Wahnideen, stabiler paranoider Verlaufstyp und Tendenz, Wahninhalte in entsprechende Handlungen umzusetzen, war die delinquente Gruppe durch das statistisch signifikant gehäufte Auftreten von antisozialen Persönlichkeitsstörungen (35% vs. 10%) und Alkoholmißbrauch (40% vs. 5%) (nicht jedoch Substanzmißbrauch insgesamt) charakterisiert. Innerhalb des delinquenten Kollektivs fand sich jedoch eine Untergruppe, die bezüglich antisozialer Persönlichkeitsstörung und Alkoholmißbrauch nicht von der nicht-delinquenten Kontrollgruppe zu unterscheiden war und ausschließlich Gewaltdelikte wie Mord oder Mordversuch begangen hatte. Die das übrige Spektrum der Delinquenz umfassende Restgruppe wies besonders häufig antisoziale Persönlichkeitsstörungen (50%), Substanzmißbrauch (71%) und Alkoholmißbrauch (50%) auf. Die Ergebnisse sind im Hinblick auf die möglichst frühzeitige Erfassung und Betreuung einer deliktgefährdeten Subgruppe Schizophrener von Bedeutung.
Summary
20 irresponsible male schizophrenic delinquents, diagnosed according to ICD-9 and DSM-III-R were compared with a control-group of 20 non-delinquent patients, matched by diagnosis, age at first hospitalisation, number and total duration of time in hospital with regard to psychopathology, antisocial personality disorder and substance abuse. Systematized delusional ideas, stable paranoid subtype and a tendency to act according to delusions were statistically overrepresented in the delinquent group as well as antisocial personality disorders (35°%% vs. 10%) and alcohol abuse (40% vs. 5%) (but not substance abuse). Among the delinquent schizophrenics a subgroup could be detected, exhibiting no increased rates of antisocial personality disorders and alcohol abuse (so comparable to the control group). Murder and attempted murder were the only delicts of this troup. The rest of the delinquent schizophrenics, showing all other kinds of delinquency, was characterized by especially high rates of antisocial personality disorders (50%), substance abuse (71%), and alcohol abuse (50%). The results are of importance in connection with early detection and treatment of a high-risk group of potentially delinquent schizophrenics.
