Zusammenfassung
Endotoxin ist heute als das entscheidende pathogenetische Prinzip gramnegativer Bakterien akzeptiert. In der Blutbahn zirkulierendes Endotoxin stellt den primären Trigger der Mediator-Kaskade dar, die im Verlaufe der gramnegativen Sepsis aktiviert wird. Bis heute steht noch keine allgemein anerkannte Anti-Endotoxin-Therapie im klinischen Gebrauch zur Verfügung. Ziel der vorliegenden Arbeit war, in vitro und in vivo eine eventuelle Endotoxin-Neutralisationswirkung verschiedener käuflicher Immunglobulin-Präparate zu untersuchen. Als in vitro System wurde der Limulus-Amöbozyten-Lysat-Test herangezogen, als in vivo System wurde ein Endotoxinschockmodell sowie ein Sepsismodell bei der Ratte etabliert. Reine IgG-Präparate zeigten im Limulus-Test keinerlei Einfluß auf die Endotoxizität, gleiches gilt für die Ergebnisse der Tierversuche in beiden Systemen. Ein IgM-angereichertes Präparat zeigte jedoch einen entscheidenden Einfluß auf die Endotoxizität, sowohl im Limulus-Test als auch im Tiermodell. Gleichfalls ist im primär letalen Sepsismodell die Überlebenszeit der IgM-behandelten Tiere deutlich verlängert im Vergleich zur Kontrollgruppe. Auch die arterielle Hypotension tritt verzögert ein. Diese Ergebnisse zeigen, daß eine Anti-Endotoxinwirkung nicht von reinen IgG-Präparaten, jedoch sehr wohl von IgM-angereicherten Präparaten zu erwarten ist.
