Zusammenfassung
Zur Implantation obigen Linsentyps wurde und wird an der I. Wiener Augenklinik prinzipiell der Faulkner-Folder verwendet. Für ein kontrolliertes Manövrieren dieser im Vergleich zu Silikonlinsen weit flexibleren Linse ist dieser Voraussetzung. Mit April 1988 sind wir von der Sulkus- auf die Kapselsackfixation übergegangen. Gründe hiefür waren die bei sulkusfixierten IOGEL-Linsen zu beobachtende Pigmentdispersion sowie mangelnde Stabilität und Zentrierung. Voraussetzung für die Wahl des Kapselsackes als Fixationsort wiederum war eine perfekte Rhexistechnik, da nur eine glattrandige Kapsulotomieöffnung eine sekundäre Dislokation und Dezentrierung sicher ausschließt. Hohe Flexibilität und Vulnerabilität der IOGEL-Linse erfordern eine besondere Handhabung während der Implantation und Fixation. Die unverzichtbar notwendige viskoelastische Substanz muß danach restlos aus dem Kapselsack wieder entfernt werden. Fehler im Detail können Komplikationen nach sich ziehen.
Eine adäquate Schnitt-, Rhexis- und Falttechnik schließt Komplikationen aus. Diese sowie die Vermeidung von Fehlern und die Beherrschung daraus resultierender Komplikationen werden besprochen.
Summary
Technique of small-incision implantation and capsular-bag fixation for the flexible pHema intraocular lens IOGEL R PC-12. At the First University Eye Hospital Vienna we have been using the Faulkner folder for implanting flexible pHema IOGEL PC-12 lenses. This instrument is a prerequisite for controlled maneouvering of such a type of lens which is far more flexible than silicone lenses. As of April 1988, we have preferred placing the lens into the capsular bag. Following sulcus fixation, we observed pigment dispersion as well as lack of stability and decentration. Fixation in the capsular bag, however, requires a perfect rhexis technique since only a capsulotomy with smooth rims will exclude secondary dislocation and decentration. The flexibility and vulnerability of the IOGEL lens require adequate handling during implantation and fixation. Following lens placement, the viscoelastic substance, indispensable for this kind of surgery, has to be thoroughly removed from the capsular bag. Even “minor” technical inadequacies may lead to postoperative complications. Adequate incision, rhexis and folding techniques, however, exclude complications. The authors present details on how to avoid mistakes and how to cope with problems — should they occur.
