All blood samples were venous. Fasting values and tolerance loads were estimated between 8.0 and 10.0 a.m. and in every case were undertaken after 12 hours abstinence from any oral and intravenous food intake. If a full normal diet were not possible, the feeding of the patients was achieved in a standardized manner with intravenous and tube feeding with a total intake of circa 2,400 calories, which was divided between carbohydrates, fat and protein in the proportion 60: 20: 20.
