Zusammenfassung
Eine Analinkontinenz, vor allem eine nach einer Analatresie, kann durch eine Plastik aus dem Musculus gracilis (Pickrell) weitgehend gebessert werden. Bei dieser Methode wird der Muskel der einen Seite mobilisiert, subkutan um den Anus geleitet und am kontralateralen Sitzbeinknorren als Widerlager fixiert.
Es wird eine Modifizierung dieser Methode beschrieben, indem aus beiden Seiten nur die proximale Hälfte des Muskel mobilisiert, von oben her mit den Anus verlagert und hinter dem After miteinander vernäht wird. Daraus entsteht eine Gracilisschlinge von beiden Seiten. Diese Methode erscheint aus anatomischen und auch operationstechnischen Gründen empfehlenswert. Auch die elektromyographischen Nachuntersuchungen zeigen sowohl beim Ruhetonus, bei der Willkürinnervation und der Luftinsufflation bessere Resultate, als nach der konventionellen einseitigen Plastik. Eindeutig konnte nachgewiesen werden, daß der transplantierte Musculus gracilis „umfunktioniert“ wird und die Rolle eines Schließmuskels im Kindesalter übernehmen kann. Dies führt im besten Fall natürlich nur zu einer relativen Kontinenz, weil die Voraussetzungen für die völlige Kontinenz meist fehlen (Sensibilität): doch resultiert klinisch eine weitgehende Besserung.
Summary
Anal incontinence, especially following anal atresia, can be greatly improved through a plastic from the musculus gracilis (Pickrell). This method mobilizes the muscle on one side, which is then placed subcutaenously round the anus and affixed on the contralateral os ischii as a counter-balance.
A modification of this method is described, whereby only the proximal half of the muscle is mobilized from both sides, placed round the anus from above and stitched together behind the anus. The result is a gracilis loop from both sides. This method would appear to be preferable for anatomical reasons as well as for reasons of operative technique. Subsequent electromyographic examination also showed better results both in relaxed tonus, in voluntary innervation and insufflation of air than does the conventional unilateral plastic. It could be demonstrated beyond any doubt that the transplanted musculus gracilis adapts to its new function and takes over the role of the spincter in childhood. Naturally, this only leads to a relative continence even in optimal cases, because the prerequisites for perfect continence are usually missing (sensibility), it does, however, result in a definite clinical improvement.
