Abstract
Two kinds of surgical approach via the skull base were introduced for meningiomas arising around the cavernous sinus. The aim of those approaches are to minimize brain retraction, having a wide exposure for radical surgery. The transzygomatic approach was applicable for orbit-cranial boundary meningiomas extending to the cavernous sinus. The zygomatic arch, orbital wall and middle cranial base were removed in the procedure. The transpetrosal approach was beneficial for meningiomas arising from clivus or anterior pyramidal bone, and extending to the posterior cavernous sinus. The anterior pyramidal bone was resected extradurally through the temporal craniotomy. Nineteen surgical examples and results are mentioned in this study.
