Zusammenfassung

Trotz bedeutender Fortschritte auf dem Gebiet der Therapie des Bronchuscarcinoms ist insbesondere die Behandlung der fortgeschrittenen oder rezidivierten Stadien unbefriedigend. Neben Verbesserungen der chirurgischen, radiotherapeutischen und zytostatischen Therapien kommen auch zunehmend biologische Therapien zum Einsatz, die sich jedoch alle derzeit noch in einem experimentellen Stadium befinden. Unser zunehmendes Verständnis über molekularbiologische Grundlagen der Entstehung und des Wachstums von malignen Tumoren hat uns gezeigt, daß ein großer Teil der Bronchuscarcinome mit bestimmten Genveränderungen einhergeht. Im derzeitigen Stadium wird versucht, aus der Analyse solcher molckularbiologischer Marker eine genauere Tumorcharakterisierung und damit eine exaktere Prognose und im Idealfall prädiktiv eine spezifische Therapiestrategie zu erstellen. Die Entwicklung der rekombinanten DNA-Technologie gibt uns jedoch auch die Möglichkeit, direkt in die Regulation dieser Tumorentstehung und -ausbreitung einzugreifen und damit Defekte zu korrigieren oder zu kompensieren. Die Ergebnisse der grundlagenwissenschaftlichen Forschung beginnen nun auch Eingang in die Klinik zu finden. Auch in der Behandlung des Bronchuscarcinoms sind erste Ergebnisse über experimentelle Therapien vorhanden, allerdings ergeben sich daraus noch keine klaren Indikationen. Im folgenden soll in einer kurzen Übersicht auf die Ergebnisse experimenteller Therapieansätze eingegangen werden, wobei allerdings aufgrund der Vielfalt der Möglichkeiten kein Anspruch auf Vollständigkeit erhoben wird.
