Abstract

Auch nach 40-jährigem Einsatz von Antidepressiva in Klinik und Praxis bleibt die Frage nach der adäquaten, optimalen Dosierung dieser Substanzen im Einzelfall häufig offen. Nur etwa 50-60% der Patienten sprechen auf ein Antidepressivum an, his zu 30% gelten als „herapieresistent” (DINAN 1993, BURROWS et al. 1994). QUITKIN (1985) konstatierte, dass 30-80% der sogenannten therapieresistenten Patienten unterdosiert seien und 50% unter adäquater Dosis respondierten. SIMPSON et al. (1982) zeigten, dass fast die Hälfte der mit 200 mg Imipramin pro die behandelten Patienten keine suffizienten Plasmaspiegel aufwiesen. Während die Bestimmung von Lithium-Plasmakonzentrationen mit relativ geringem Aufwand möglich ist, wurde der zuverlässige Nachweis von Antidepressiva in Anbetracht der sehr geringen Konzentrationen erst in den letzten Jahren durch die Entwicklung aufwendiger Methoden möglich. So lässt sich die Beziehung zwischen Plasmakonzentration und Nebenwirkungen einerseits und therapeutischer Wirksamkeit andererseits gezielt erforschen. Bedingt durch die Komplexität der klinischen und pharmakokinetischen Einflussgrößen sind hierbei allerdings eine Vielzahl von Variablen und Parametern zu beachten (Übersichten: BAU-MANN 1992, LAUX und RIEDERER 1992, HIEMKE et al. 2000).
