Zusammenfassung
Bei Verschlüssen viszeraler Gefäße kommt es in Abhängigkeit vom Ausmaß des Verschlusses bzw. der bestehenden Kollateralisation zum Auftreten von akuter oder chronischer mesenterialer Ischämie. Unbehandelt führt eine akute mesenteriale Ischämie innerhalb von einigen Stunden zu irreversiblen Durchblutungsstörungen mit nachfolgender Darmnekrose und Peritonitis. Die akute mesenteriale Ischämie kann durch embolische (50%) oder thrombotische Verschlüsse (15%) viszeraler Arterien, durch Thrombosen im Bereich des viszeralen Venensystems (15%) sowie als sog. nichtokklusive Ischämie (20%) auftreten. Dem akuten Viszeralarterienverschluss liegt in den meisten Fällen (ca. 85%) ein akuter Verschluss der A. mesenterica superior, gefolgt von Verschlüssen im Bereich des Truncus coeliacus und der A. mesenterica inferior zugrunde. Unter den akuten Viszeralarterienverschlüssen stellt die nichtokklusive mesenteriale Ischämie (nichtokklusive ischämische Enteropathie, NOMI, NOD) basierend auf einer spastischen Perfusionsstörung im mesenterialen Stromgebiet einen Sonderfall dar.
