Zusammenfassung
Der psychischen Dimension wird auch in der Herzchirurgie immer mehr Beachtung geschenkt – und dies mit gutem Grund. Herzchirurgische Eingriffe sind zu einem großen Prozentsatz mit einer psychischen Überforderung der betroffenen Patienten verknüpft. Von diesen zeigen ca. 40 % Symptome einer Depression und damit verbunden eine ängstlich-depressive bis pessimistische Einstellung (Pignay-Demaria et al. 2003). Diese resultiert einerseits aus der Angst vor der Operation, besonders vor der Brustkorberöffnung, und andererseits auch aus der Angst vor den Folgen des Eingriffs. Dazu kommen zusätzlich noch Sorgen wegen einer möglichen zukünftigen Einschränkung der Lebensqualität. Sind die psychischen Probleme gravierend, so ist der psychiatrische Konsiliardienst zu Hilfe zu rufen. Die psychosomatischen Aufgaben müssen sich das herzchirurgische Team und das Konsiliarteam teilen. Sowohl vom behandelnden Team, besonders dem Operateur, als auch vom Psychiater wird ein sehr persönliches Eingehen auf die individuelle Situation des Patienten gefordert. Es gilt der Grundsatz: „Hole den Patienten ab, wo er steht und führe ihn dorthin, wo er stehen soll“. Im Rahmen des Aufklärungsgesprächs ist das Ausmaß der psychischen Irritation und der Angstzustände, aber auch eventuelle kognitive Defizite zu erheben. Dies ist besonders bei ängstlichen Patienten mit einem erhöhten Zeitaufwand verbunden, der einberechnet werden muss. Durch das individuelle Erfassen der Situation des Patienten entstehen ein Vertrauensverhältnis und eine Klarheit, die sich positiv auf den Stimmungszustand des Patienten auswirken. „Angst essen Seele auf“ ist nicht nur ein schöner Filmtitel von Rainer Werner Fassbinder, sondern beschreibt auch sehr eindrucksvoll die Situation eines Patienten, der nicht ausreichend über das Ausmaß der Erkrankung, die Art der bevorstehenden Operation und die Prognose informiert ist.
