Der Prozess bis zur Erstellung des fertigen Befundes wird in drei Schritte (Abb. 2.1) unterteilt:

Postanalytik (Interpretation) Unter Präanalytik fallen alle Vorgänge, die bis zur Messung stattfinden können. Gerade in dieser Phase kann viel passieren, was den Befundwert beeinflussen kann. Von allen „Laborfehlern“ fallen 32–75 % in die präanalytische Phase (31–16% analytische Phase; 9–30% postanalytische Phase). Insgesamt sind von derartigen „Laborfehlern“ 0,33–0,61% der Befunde betroffen. Das entspricht in etwa 0,5–0,11 % aller Patienten. Diese Laborfehler haben in 43–74 % der Fälle keine Folgen. In 13–23 % kommt es zu milden Folgen (Zeitverzögerung, Folgeuntersuchungen), in 6,4–50 % kommt es zu moderaten Folgen (möglicher Schaden, inadäquate Therapie) in 6,4–25 % kann es jedoch zu schwerwiegenden Folgen im Sinne von falschen medizinischen Interventionen kommen (1). Um derartige Fehler vermeiden zu können, ist es notwendig, die Ursachen dafür zu kennen.
