Hyperlipoproteinemia in type II-diabetics: an investigation into prevalence, incidence and results of treatment

Abstract

In the Diabetes intervention study, we investigated 1139 patients (aged 30–55 years) with newly manifested, dietetically compensated type II-diabetes. The patients were randomly allotted to 2 intervention groups with intensified health education plus clofibric acid or placebo (double blind manner) and a control group. At the time of clinical manifestation of the diabetes we found in 17,6% of the patients a primary HLP (hyperlipoproteinemia), whereby the hypertriglyceridemia dominated.

After 5 years diabetes duration TC had significantly risen in all groups, whereas TG concentration could be influenced through the intensified health education and the clofibric acid therapy. At this time significant relations between the TG values and signs of CIHK in resting ECG could be demonstrated. After 5 years patients with higher lipid values have shown a more unfavourable risk profile in relation to RR, IGI and FBG (fasting blood glucose) than patients with lower lipids.

Zusammenfassung

Diabetiker sterben infolge makroangiopathischer Gefäßkomplikationen im Vergleich mit der Normalbevölkerung auch heute noch früher. Die Prognose für Diabetiker verschlechtert sich besonders durch das Vorliegen weiterer Risikofaktoren erster Ordnung, wie der Hypertonie und der Hyperlipoproteinämie, die bei Diabetikern unverhältnismäßig häufiger vorkommen als bei der Normalbevölkerung. Bisher gibt es nur relativ wenige prospektive Studien, die über den Verlauf des Typ II-Diabetes und das Auftreten makroangiopathischer Gefäßkomplikationen Auskunft geben können.
