Abstract
In a prospective study, 544 patients of a rehabilitation clinic for stroke patients were studied in regard to neurological and haemorheological parameters. The type, expression and severity of the apoplexy was evaluated using a score index modified after the Toronto Stroke Scale and the Canadian Neurological Scale, and patients were separated into sub-groups accordingly. It could be demonstrated that following the end of the acute phase, patients with ischaemic apoplexy had a worse rheological profile than patients with apoplexy of a different type (p-values ca. 0.05). There was a positive correlation between the extent, of residual symptoms and the risk of reinfarction within the period of investigation of two years (X2=0.0002). This risk is even greater for patients where the observed apoplexy was already a relapse (X2=<0.00001). This was valid irrespective of whether patients had suffered an ischaemic insult or an apoplexy of a different type.
Zusammenfassung
In einer prospektiven Studie wurden 544 Patienten einer Rehabilitationsklinik für Schlaganfallpatienten auf neurologische und hämorheologische Parameter untersucht. Anhand eines modifizierten Scores auf der Basis der Toronto Stroke Scale und der Canadian Neurological Scale wurden Art des Apoplexes, Ausprägung und Schweregrad bewertet und die Patienten verschiedenen Untergruppen zugeordnet.
Es zeigte sich, daß auch nach Abklingen der Akutphase Patienten mit ischämischem Apoplex ein schlechteres rheologisches Profil haben als Patienten mit einem Apoplex anderer Genese (p-Werte um 0.05).
Eine positive Beziehung fand sich zwischen dem Ausmaß der Residualsymptomatik und dem Risiko für einen Reinfarkt im Untersuchungszeitraum von zwei Jahren (X2 = 0.0002). Noch größer ist dieses Risiko für Patienten, wenn es sich bei dem beobachteten Apoplex bereits um ein Rezidiv handelt (X2 < 0.00001). Dies gilt gleichermaßen für Patienten mit ischämischem Insult und für Patienten mit einem Apoplex anderer Ursache.
