Zusammenfassung
Es wird aufgezeigt, daß der Begriff Methylxanthine für Theophyllin und synthetische Xanthin-Derivate mißverständlich verwandt wird.
In der Theophyllin-Literatur wird häufig nicht ausreichend unterschieden zwischen Salzen und chemischen Derivaten desselben. Dadurch wird der Eindruck erweckt, daß aus allen sogenannten bronchospasmolytisch wirkenden Theophyllinen in jedem Fall metabolisch oder hydrolytisch Theophyllin entsteht. Dies gilt jedoch allein für Salze des Theophyllins, nicht aber für chemische Derivate, die sich vom Theophyllin ableiten.
Anhand des synthetischen Xanthin-Derivates Pentoxifyllin, einem Vasotherapeuticum, das sich chemisch vom Coffein ableiten läßt, wird aufgezeigt, daß durch chemische Derivatisierung nicht nur physikalisch-chemische Eigenschaften, wie die Wasser- und Lipoidlöslichkeit, sondern auch die pharmakokinetischen, wie Resorptionsgeschwindigkeit, Eiweißbindung, Verteilungsräume und die Höhe der therapeutischen Plasmaspiegel verändert werden. Offensichtlich durch die deutlich verschiedenen physikalisch-chemischen Eigenschaften des Pentoxifyllins im Vergleich zu den natürlich vorkommenden Methylxanthinen ändern sich auch die metabolischen Abbauwege. Während Pentoxifyllin im wesentlichen über die Oxohexylseitenkette metabolisiert wird, unterliegen z. B. Theophyllin und Coffein im Körper Demethylisierungsreaktionen, einer Oxydation zu entsprechenden Methyl-Harnsäuren sowie weiteren biologischen Abbaureaktionen. Für Theophyllin liegen die therapeutischen Plasmaspiegel in Form eines schmalen Fensters zwischen 10 und 20 µig/ml, die des Pentoxifyllins dagegen unter 0,5 µg/ml. Pentoxifyllin besitzt weiterhin im Vergleich zu Coffein, Theophyllin und Theobromin andere pharmakologische Eigenschaften und zeichnet sich durch eine günstigere Verträglichkeit aus.
Summary
It is demonstrated that the expression methylxanthines as used for theophylline and for synthetic xanthine derivatives is misleading. In publications concerning theophylline there is often no clear distinction between salts of theophylline and chemical derivatives of theophylline. The impression is thus given that all so-called bronchospasmolytic active theophylline salts and derivatives are converted to theophylline by metabolic or hydrolytic processes. This, however, is only true for salts of theophylline and not for its chemical derivatives. In the case of the synthetic xanthine derivative pentoxifylline, a vasotherapeutic agent of the caffeine type, it is shown that on chemical derivatization not only are physico-chemical parameters, for example water- und lipid-solubility changed, but also pharmacokinetic parameters, such as resorption rates, distribution volumes, and therapeutic plasma levels. The different physico-chemical properties of pentoxifylline in comparison to natural methylxanthines, result in a change in the metabolic pathway. While pentoxifylline is mainly metabolized via oxidation of the oxohexyl side chain, theophylline and caffeine are metabolized mainly by demethylation reactions. The therapeutic plasma levels for theophylline are between 10 and 20 µg/ml whereas those of pentoxifylline are below 0.5 µg/mi. Furthermore, pentoxifylline exhibits different pharmacological properties in comparison to caffeine, theophylline and theobromine and is better tolerated by the patient.
