Dosage and tolerance of antacids

Abstract

Antacids contain anions such as hydroxide, bicarbonate or silicate, and cations such as aluminium, magnesium or calcium. As such they could influence the mineral balance of the body, especially if there is a renal malfunction. An aluminium excess in dialysis patients can lead to encephalopathy. At times the pathogenesis of Alzheimer’s disease has been linked with the intake of aluminium. But today we know that this disease is most likely to be caused by a genetic defect. Impairment of the calcium-homeostasis can lead under the therapy with calcium-containing antacids to hypercalcaemia and/or to the development of nephrolithiasis. It is therefore advised that antacid administration is reduced to the minimum effective dose. It has been shown that a daily dosage of as little as 120 – 150 mEq of antacids is sufficient to achieve optimal therapeutic effectiveness. This is well below the necessary neutralisation equivalent, and point to additional effects of antacids, other than acid neutralisation.

Zu den Antacida gehören auf der anionischen Seite Hydroxide, Karbonate und Trisilikat, auf der kationischen Seite Aluminium, Magnesium und Kalzium. Sie können den Mineralhaushalt des Körpers erheblich beeinflussen, vor allem, wenn eine Nierenfunktionsstörung vorliegt. Hohe Aluminiumgaben können bei dialysepflichtigen Patienten zu Enzephalopathien führen. Die Pathogenese des Morbus Alzheimer wurde zeitweise mit Aluminium in Verbindung gebracht. Heute geht man davon aus, daß diese Krankheit auf einem defekten Gen beruht. Störungen der Kalziumhomöostase können durch eine Antacidatherapie zu Hyperkalzämie und/oder zu Nierensteinen führen. Antacidagaben sollten daher auf die minimal wirksame Dosis reduziert sein. Es konnte gezeigt werden, daß eine Tagesdosis von 120 – 150 mval für eine optimale therapeutische Wirkung ausreicht. Diese Dosis liegt deutlich unter dem erforderlichen Neutralisierungsäquivalent, was auf Wirkungsmechanismen der Antacida hinweist, die über die Säureneutralisierung hinausreichen.
