Abstract
Antacids play an integral role in the treatment of diseases of the upper gastrointestinal tract. Correctly applied they lead to a notable reduction in intragastral acidity, which seems to be the basis for their effectiveness. More recently, additional mechanisms of action have been demonstrated, such as the binding of bile acids in the case of aluminium hydroxide containing antacids. Furthermore, the mucosa protecting mechanisms are now considered part of an efficient antacid therapy. Chemically, antacids can be divided into anionic and cationic compounds. In their clinical application it is necessary to combine different antacids taking advantage of their various acid-binding properties in order to achieve an optimal therapy. In this context aluminium hydroxide and magnesium hydroxide have proved to be of great importance both as starting materials for the production of antacids as well as component partners in other combinations.
Zusammenfassung
Antacida sind nach wie vor integraler Bestandteil der ärztlichen Therapie bei Erkrankungen des oberen Gastrointestinaltraktes. Bei korrekter Anwendung bewirken sie eine maßvolle Reduktion der intragastralen Azidität, die ihre Wirksamkeit maßgeblich bestimmt. Zusätzliche Wirkmechanismen, wie eine insbesondere für aluminiumhydroxidhaltige Antacida relevante Gallensäurenbindung, sind in jüngerer Zeit nachgewiesen worden. Der Stellenwert mukosaprotektiver Mechanismen für eine effiziente Antacidatherapie wird seit neuestem diskutiert. Aus chemischer Sicht sind Antacida in anionische und kationische Verbindungen zu unterteilen. Für die klinischen Anwendungen sind Kombinationen von Antacida notwendig, die die unterschiedlichen Säurebindungseigenschaften der einzelnen antaciden Komponenten optimal ausnutzen. Hierbei sind Aluminium- und Magnesiumhydroxid sowohl als Ausgangsstoffe für antacide Verbindungen als auch als Kombinationspartner von überragender Bedeutung.
