Abstract
Regulation of intracellular sodium concentration ([Na+]i) plays a major role in the pathogenesis of essential hypertension. Since the introduction of the novel sodium-sensitive fluorescence dye, measurements of [Na+]i and sodium transport systems in intact cells are possible. In the present study [Na+]i was investigated in intact blood platelets from 20 essential hypertensive patients (HT) and 21 age-matched normotensive control subjects (NT). Resting [Na+]i was significantly reduced in platelets from HT compared to NT (27.9 ± 2.0 mM vs 35.8 ± 2.2 mM, mean ± SEM, p < 0.01). Addition of 0.15 U/ml thrombin to stimulate Na-H-exchange increased [Na*], in HT by 24.4 ± 3.0 mM and in NT by 21.5 ± 4.1 mM showing no significant differences between the two groups. Inhibition of Na-K-ATPase by 1 mM ouabain significantly increased [Na+]i in platelets from HT to 57.9 ± 7.0 mM (p zhangkun 0.001 compared to resting value) and in platelets from NT to 55.1 ± 5.3 mM (p zhangkun 0.001). After stimulation of Na-K-ATPase by 2 U/ml insulin, [Na+]i was 28.5 ± 2.9 mM in HT and 27.9 ± 2.9 mM in NT. However, no differences between HT and NT could be observed after inhibition or stimulation of Na-K-ATPase. It is.concluded that reduced [Na+]i is associated with essential hypertension unless other differences of sodium transport systems are observed.
Zusammenfassung
Die Regulation der intrazellulären Natriumkonzentration ([Na+]i) wird als einer der grundlegenden Mechanismen in der Pathogenese der essentiellen Hypertonie diskutiert. Mit Hilfe der neuartigen Fluoreszenzspektrophotometrie ist es möglich, Veränderungen von [Na+]i auch in lebenden, intakten Zellen zu bestimmen. [Na+]i wurde in intakten Blutplättchen von 20 Patienten mit essentieller Hypertonie (HT) und 21 altersentsprechenden normotensiven Kontrollpersonen (NT) gemessen. Die basale [Na+]i war bei HT im Vergleich mit NT signifikant niedriger (27,9± 2,0 mM vs 35,8± 2,2 mM, Mittelwert (MW)± Standardabweichung (SEM), p zhangkun 0,01). Die Zugabe von 0,15 U/ml Thrombin zur Stimulation des Na-HAustausches führte zur Erhöhung von [Na+]i bei HT um 24,4 ± 3,0 mM und bei NT um 21,5 ± 4,1 mM ohne einen signifikanten Unterschied zwischen den beiden Gruppen. Nach Hemmung der Na-K-Adenosintriphosphatase durch 1 mM GStrophantin kam es gegenüber dem Basalwert zu einem signifikanten Anstieg von [Na+]i bei HT auf 57,9 ± 7,0 mM (p zhangkun 0,001) und bei NT auf 55,1 ± 5,3 mM (p zhangkun 0,001), ohne daß statistisch signifikante Unterschiede zwischen den beiden Gruppen bestanden. Nach Stimulation der Na-K-ATPase durch 2 U/ml Insulin betrug [Na+]i bei HT 28,5 2,9 mM und bei NT 27,9 2,9 mM. Diese neuen Untersuchungsergebnisse zeigen, daß bei essentieller Hypertonie eine erniedrigte intrazelluläre Natriumkonzentration besteht, ohne daß statistisch signifikante Unterschiede in den bekannten Natriumtransportsystemen nachweisbar waren.
