Zusammenfassung
Erwartungen über die Entwicklung und Phasen schizophrener Symptomatik werden von den traditionellen Konzepten auf der einen Seite durch Attribute wie »prodromal«, »aktiv« und »residual« beschrieben, auf der anderen durch das Konzept »positiver« und »negativer« Symptome. Drei Dimensionen der Symptomatologie, nämlich Positivsymptome, zu welchen Halluzinationen und Wahnphämonene gehören, Negativsymptome und Dysphorie (mit Angst und depressiven Symptomen) sind bei verschiedenen Gruppen von Patienten unterschiedlich bedeutsam für die Entwicklung von »Chronizität« oder sozialer Behinderung. Die Prognose hinsichtlich chronischer Entwicklungen wird nicht allein vom Ausmaß der Negativsymptomatik beeinflußt, sondern auch durch die (dynamischen) Proportionen von Positiv- und Negativsymptomatik des Patienten im Verlauf der Psychose.
Das Ergebnis dieser drei Dimensionen ist konsistent mit dem Kontinuum-Modell der Krankheitsausgänge (»recovery«), welches die Symptome in drei breiten Gruppen anordnet, nämlich in »positive«, »negative« und »unspezifische« (oder »dysphorische«, d. h. Depressions- und Angst-)Symptome (Carr, 1983). Insgesamt dominieren die negativen Symptome gegenüber den positiven in ihrem Einfluß auf die Chronizitätsentwicklung, was relativ lang andauernde soziale Behinderungen impliziert.
Dieser Beitrag basiert auf Daten, die innerhalb der WHO Collaborative Study »On the Assessment and Reduction of Psychiatric Disability« in Mannheim erhoben wurden. Die Studie wurde unter der Koordination von A. Jablensky (WHO/Genf) durchgeführt. Als weitere Zentren nahmen an der Studie teil: Ankara, Groningen, Karthoum, Sofia, Zagreb und Zürich.
Der am Zentralinstitut für Seelische Gesundheit in Mannheim durchgeführte Teil der Studie wurde in der Zeit von 1976 bis 1981 vom Bundesministerium für Jugend, Familie und Gesundheit gefördert, ab 1983 durch das Bundesministerium für Forschung und Technologie.
