Zusammenfassung

Der Verlauf der Schizophrenie ist unterschiedlich. Eine besondere Gruppe chronischer Fälle — die derzeitige Population entspricht in etwa der Gruppe, die vor Einführung der Neuroleptika Anfang der fünfziger Jahre in den psychiatrischen Krankenhäusern war — zeigt verschiedene Formen einer mehr oder weniger schweren Behinderung in medizinischer wie sozialer Hinsicht. Ziel dieses Symposiums über eine Standortbestimmung (»reconnaissance workshop«) ist es, Behandlungsstrategien für diese Gruppe von schwerkranken Patienten zu diskutieren.

Wir sind davon überzeugt, daß viel mehr für diese chronisch schizophrenen Patienten getan werden könnte, wenn bestimmte therapeutische Prinzipien akzeptiert und in der Praxis angewandt würden. Der folgende Beitrag bringt eine kurze Hintergrundbeschreibung des Problems mit Patientencharakteristika, den verschiedenen Einstellungen zu diesem Problem, den wichtigsten Gesichtspunkten der Weiterbehandlung und den zu verwendenden Beziehungsgrößen, um Wirksamkeit und Qualität der Behandlung beurteilen zu können.
