Zusammenfassung

Die physische Aktivität ist der Promotor der gesunden Ontogenese, und der Muskelstatus prägt in allen Lebensphasen den Gesundheitszustand. Die Inaktivität mündet in chronisch degenerativen Erkrankungen. Es werden Vorstellungen zu den grundsätzlichen Schritten der Pathogenese bis hin zur chronischen Schmerzerkrankung dargestellt. Die inaktivitätsbedingte „persistent low grad inflammation“, die dekonditionierungsbedingt inadäquate Mikrozirkulation und sensomotorische Defizite werden als die „Vorläufer“ für die peripheren gewebespezifischen Krankheitsprozesse, das Entstehen der peripheren Ursachen von Schmerzen und die Entstehung der Defizite und Dysbalancen der Schmerzhemmung angesehen. Zu den primär nicht entzündlichen Erkrankungen des Stütz- und Bewegungsapparates gehören auch immer Entzündungsprozesse, und die primär entzündlichen Gelenkerkrankungen werden auch immer mit Arthroseprozessen begleitet. Beide Krankheitsprozesse ergänzen sich bei der zentralen Sensibilisierung.
