Zusammenfassung

Ermüdende Belastungen sind wichtige Elemente der Schmerztherapie. Die „exercise induced hypoalgesia“ ist ein Therapiereiz. Bei chronischen Schmerzen kann sie gemindert, nicht vorhanden oder paradox sein. Belastungschmerzen haben einen maladaptiven Hintergrund, und gesundes Altern ist keine Schmerzursache.

Die Muskulatur prägt die Körperhomöostase, den Stoffwechsel, das Gehirn und die Schmerzen. Schmerzen verändern die kognitiv-mentalen Funktionen wie die Compliance und Resilienz. Jede Belastungsform ist auch Therapie des Gehirns. Training steht sowohl für Schmerzprävention als auch für Schmerztherapie! Der Muskelstatus ist Marker des Gesundheitsstaus und der Schmerzhemmung und -toleranz.

Die pathophysiologischen und sozialen Faktoren chronischer Schmerzen prägen die Therapie. Das „nozizeptive Gehirn“ muss gefordert werden. Die Wechselbeziehungen zwischen Erkrankung, Schmerzen, Ernährung und Belastungen sind vielfältig. Dosis-Wirkungsbeziehungen der Ernährung sind zz. nicht ableitbar.
