Zusammenfassung
Die Lungentransplantation ist ein Verfahren, um das Überleben von Patienten mit verschiedenen terminalen Lungenerkrankungen zu verbessern. Sie ist mit ca. 5 Operationen auf 1 Mio. Einwohner in den westlichen Industrieländern eine seltene Therapieform.
Sehr seltene Lungenerkrankungen machen 5 % aller Indikationen zur Lungentransplantation aus, davon sind die obliterierende Bronchiolitis und die pulmonale Beteiligung bei Bindegewebserkrankungen, insbesondere der systemischen Sklerose, die Hauptindikationen.
Einige seltene Lungenerkrankungen haben eine systemische Beteiligung, die zu speziellen postoperativen Problemen nach einer Lungentransplantation führen kann. Diese systemischen Beteiligungen müssen vor einer Transplantation sehr sorgfältig geprüft werden. Typische postoperative Komplikationen bei Lymphangioleiomyomatose (LAM) z. B. umfassen den Chylothorax, chylösen Aszites und die Blutung bei Angiomyolipom. Bei der Langerhans-Zell-Histiozytose (Histiozytosis X, HX) kann es zu extrapulmonalen Manifestationen in den Knochen und in der Hypophyse kommen. Die HX und einige andere seltene Lungenerkrankungen können im Transplantat wieder auftreten, manche dieser Rezidive sind aber asymptomatisch. Die Langzeitergebnisse großer Transplantationszentren zeigen in den letzten Jahren verbesserte Ergebnisse nach Lungentransplantation, auch bei seltenen Erkrankungen. Die Haupttodesursachen nach Lungentransplantation sind das chronische Lungentransplantatversagen, Infektionen und Malignome. Die Überlebensrate bei Patienten mit seltenen Lungenerkrankungen ist denen von Patienten mit anderen Indikationen nicht unterlegen, teilweise ist sie sogar besser.
