Zusammenfassung

Das Swyer-James-Macleod-Syndrom (SJMS), ein einseitiges Lungenemphysem wahrscheinlich als Folge von Atemwegsinfekten in der Kindheit, ist gekennzeichnet durch eine einseitige Hypertransparenz im Röntgenbild und exspiratorisches Air Trapping. Klinisches Bild und Prognose hängen in erster Linie von der Ausbildung sakkulärer Bronchiektasen ab: Wiederholte Atemwegsinfekte können letztlich eine chirurgische Therapie erfordern. Bei zufälliger Diagnose im Erwachsenenalter bestehen kaum Beeinträchtigungen. Die Computertomografie des Thorax ist die diagnostische Methode der Wahl, zum Ausschluss einer endobronchialen Obstruktion einschließlich Fremdkörperaspiration wird im Kindesalter evtl. eine Bronchoskopie erforderlich sein.
