Zusammenfassung
Das Birt-Hogg-Dubé-Syndrom (BHDS) ist eine seltene, autosomal-dominant vererbte Erkrankung, die durch das Auftreten von dermalen Fibrofollikulomen, Nierentumoren und pulmonalen Zysten charakterisiert ist. Der genetische Defekt ist durch eine Mutation des Chromosoms 17 verursacht, welches das Tumorsuppressor-Gen Follikulin (FLCN) kodiert. Die Funktion des Proteins ist zum Großteil unbekannt. Es wird überwiegend in Haut, Typ-1-Pneumozyten und distalen Nephronen der Niere exprimiert und scheint in die AMPK-mTOR-Achse involviert zu sein. Patienten können sich sowohl mit Hauterscheinungen als auch Spontanpneumothoraces oder malignen Nierentumoren präsentieren. Die Variabilität der klinischen Ausprägung macht die Diagnosestellung anspruchsvoll. Die Verfügbarkeit der Mutationsanalyse ist ein wichtiger Faktor in der Diagnostik und hat einen Einblick in die unterschiedlichsten Variationen der FLCN-Expression sowohl innerhalb einer als auch zwischen verschiedenen Familien ermöglicht.
