Zusammenfassung
Die IgLON5- Enzephalitis ist eine Tauopathie mit gleichzeitigem Nachweis von IgLON5-Antikörpern im Blut und zumeist im Liquor. In der Mehrzahl der Fälle sind Männer im höheren Alter (6.–8. Lebensdekade) betroffen. Klinisch besteht eine heterogene Beschwerdesymptomatik mit Gangstörung, Schlafattacken sowie eine Hirnstammsymptomatik. In einzelnen Fallberichten werden jedoch auch Ataxie, Chorea, kraniozervikale Dystonien, Myokymien, kognitive Einschränkungen mit Halluzinationen sowie Zeichen der neuromuskulären Übererregbarkeit (z.B. Faszikulationen) beschrieben. Daneben stellen zentrale Ventilationsstörungen eine lebensbedrohliche Komplikation dar. Bislang existiert keine standardisierte Therapie. Probatorisch sollten Kortikosteroide als auch intravenös applizierte Immunglobuline eingesetzt werden.
