Zusammenfassung
Die Terrible-Triad-Verletzung beinhaltet eine meist anterior gelegene Radiuskopffraktur, eine meist transversal verlaufende Fraktur des Processus coronoideus im Sinne eines O‘Driscoll-1.2-Frakturverlaufes sowie ligamentäre Zusatzverletzung lateral und sehr häufig auch medial. Über einen lateralen anterior gelegenen transtendinösen Zugang mit Ablösung des Musculus brachialis vom Flumerusschaft können die laterale Bandläsion, die Radiuskopffraktur und die Processus-coronoideus-Fraktur komplett eingesehen und versorgt werden. Wenn der Radiuskopf osteosynthetisch nicht stabilisiert werden kann, besteht die Indikation für eine Prothese, damit die Zusatzverletzung, insbesondere also die Processus-coronoideus-Fraktur und die Bandläsion ausheilen. Nach Abschluss der Versorgung von lateral sollte die mediale Stabilität geprüft werden und diese entweder offen oder indirekt über einen Fixateur adressiert werden.
