Zusammenfassung
Bei der extrinsischen, meistens posttraumatischen Ellenbogenkontraktur kommt es nach mehr oder weniger ausgedehnten Verletzungen (Radiusköpfchenfraktur, Ellenbogenluxation, Kapselruptur oder chondrale Läsionen) zu einer Vernarbung der kapsuloligamentären Strukturen mit Elastizitätsverlust und damit zu einer Einschränkung der Streck- und Beugefähigkeit des Ellenbogengelenkes. Bei der offenen Arthrolyse über den lateralen oder radialen Säulenzugang kann über eine einzige Hautinzision im anterioren Intervall eine ventrale Kapselexzision mit Gelenkrevision und Resektion von Osteophyten am Proc. coronoideus und im posterioren Intervall eine dorsale Kapselexzision mit Resektion der Olecranonspitze erfolgen. Bei diesem Zugang wird das laterale Kollateralband erhalten, um keine dorsoradiale Instabilität zu erzeugen.
