Zusammenfassung

Die entzündlich-rheumatischen Erkrankungen haben als muskuloskelettale Leitsymptome Schmerzen, Schwellungen und Funktionseinschränkungen der Gelenke, der Wirbelsäule, Muskeln, Sehnenansätze und Sehnenscheiden. Als autoimmune Systemerkrankungen betreffen sie aber auch Blutgefäße (Vaskulitiden) und können sich zudem an der Haut, den Augen, dem Nervensystem und nahezu allen inneren Organen manifestieren. Kardiale Beteiligungen sind häufig, wenn man Rheumapatienten systematisch mit sensitiven Techniken wie Echokardiografie, CT oder MRT untersucht oder Autopsiebefunde zugrunde legt. Klinisch manifeste Herzmanifestationen sind hingegen seltener, können allerdings als Erstsymptom auf eine schwerwiegende entzündliche Systemerkrankung auch junger Menschen hinweisen und Anlass für aggressive immunsupprimierende Therapien sein. Labormarker einer Systementzündung sind fast immer erhöht; spezifische Autoantikörper (Rheumafaktor, antinukleäre Antikörper u. a.) helfen, die Erkrankung zu klassifizieren.
