Zusammenfassung
Synkope ist ein häufiges, bisweilen lebensbedrohliches Krankheitsbild. Von besonderer Bedeutung sind die initiale Basisdiagnostik, die Abgrenzung synkopenähnlicher Erkrankungen und die Risikoeinschätzung bzgl. möglicher lebensbedrohlicher Ursachen, insbesondere Arrhythmien. Hiervon wird die spezifische weiterführende Diagnostik und Therapie bestimmt. Bei fehlenden Hochrisikokriterien sind die wichtigsten diagnostischen Untersuchungen: die Kipptischuntersuchung bei V. a. Reflexsynkopen (falls nicht bereits anamnestisch geklärt) oder die längerfristige Rhythmusüberwachung mittels implantierbarem Loop-Rekorder (ILR) bei begründetem Verdacht auf eine arrhythmogene Synkope (ungeklärte, rezidivierende Synkopen ohne Prodromi). Mittels Echokardiografie und ggf. Ischämiediagnostik sollte geklärt werden, ob eine Herzerkrankung vorliegt. Therapieoptionen umfassen bei Reflexsynkopen Patienteninformation, isometrische Manöver, Stehtraining, erhöhte Trinkmenge, selten medikamentöse Therapie, extrem selten Schrittmachertherapie.
