Zusammenfassung

Eine Myokarditis kann sowohl im Rahmen von Infektionen als auch infolge von nichtinfektiösen Erkrankungen wie z. B. der Sarkoidose entstehen. Bei einem Großteil der durch Infektionen bedingten Myokarditiden kommt es zu einer spontanen Ausheilung der Erkrankung. Bei einem geringen Teil der Patienten/-innen mit Myokarditis kommt es allerdings zu einer chronisch progredienten Erkrankung mit konsekutiver Störung der myokardialen Funktion entsprechend dem klinischen Bild einer dilatativen Kardiomyopathie. Der Goldstandard für die Diagnose einer Myokarditis ist weiterhin die endomyokardiale Biopsie. Wichtig ist, dass die Myokardbiopsie histologisch, immunhistologisch und unter Verwendung molekularbiologischer Techniken auch virologisch exakt analysiert wird, um die verschiedenen Entitäten der Myokarditis exakt zu erfassen und dann mögliche kausale Therapieoptionen einleiten zu können. Als kausale Therapieoptionen stehen immunmodulatorische und antivirale Ansätze zur Verfügung.
