Zusammenfassung

Das vorliegende Kapitel stellt die Pychosomatik der nicht-alkoholischen Fettlebererkrankung (NAFLD) dar. Diese ist mit der Binge-Eating-Störung verbunden, aber auch mit einer psychischen Labilität, die sich in häufiger emotionaler Verstimmtheit äußern kann. Betroffene haben offenbar Schwierigkeiten, sich psychisch und emotional zu regulieren. Spannung wird daher unter Umständen fälschlich als ein Hungergefühl interpretiert, wobei dann das Essen gewohnheitsmäßig zur Spannungsregulation dient. Das Kapitel beschreibt die psychodynamischen Aspekte des Geschehens, bei dem dysfunktionale Verarbeitungsmechanismen, mangelhafte Selbststeuerung und emotionale Labilität eine Abwärtsspirale in Gang setzen. Der ärztliche Umgang mit diesen Patienten erfordert daher psychosomatische Sensibilität, zumal die Behandlung in vielen Fällen hohe Anforderungen an die Belastbarkeit der Patienten stellt. Das Kapitel unterstreicht daher die Bedeutung der Arzt-Patient-Beziehung für die Führung dieser Patienten.
