Massive and Irreparable Rotator Cuff Tears

Abstract

Irreparable massive tears of the rotator cuff are a challenge for orthopedic surgeons. Limited options exist for the treatment of these tears; although multiple treatment options have been described, some of them are insufficient to meet patient requirements, some others are also technically demanding and result in difficult rehabilitation or simply are not ideal for relatively young active population, as for reverse total shoulder arthroplasty. Complete repair of the rotator cuff and full coverage of the femoral head footprint should be the goal in any repair because of the consistent good results, but when that target is not possible, achieving a balanced force couple with partial or incomplete humeral head coverage can achieve good enough results for many patients in the mid-term and does not preclude further surgery.
