Zusammenfassung

Die avaskuläre Osteonekrose des Humeruskopfes, auch Morbus Haas genannt, wird beispielsweise durch Kortikosteroid-Therapie, durch Alkoholismus oder im Rahmen von Hämoglobinopathien induziert. Das klinische Hauptsymptom ist der Schmerz. Die klinischen Symptome beginnen meist später als im Rahmen einer Femurkopfnekrose, was zu einer verspäteten Diagnosestellung führen kann. Die für die Femurkopfnekrose entwickelte radiologische Klassifikation nach Ficat und Arlet wurde von Cruess für die Beurteilung der Humeruskopfnekrose angepasst und umfasst 5 Stadien. Die Magnetresonanztomographie mit Kontrastmittelapplikation ist die bildgebende Methode zur genaueren Einschätzung der Osteonekrose. Therapeutisch steht die Identifikation von Risikofaktoren und deren Vermeidung im Vordergrund. Die konservative, bewegungserhaltende Behandlung ist wichtig und wird ergänzt durch die operative Therapie mit der Arthroskopie, der retrograden Anbohrung und in späteren Stadien durch den endoprothetischen Gelenkersatz.
