Zusammenfassung

Der Morbus Blount ist eine knöcherne Wachstumsstörung im Kindes- und Jugendalter am posteromedialen Anteil der proximalen Epi- und Metaphyse der Tibia, die zu einer typischen progressiven O-Bein-Deformität (Genu varum) führt. Es wird zwischen einer infantilen, juvenilen und adoleszenten Form unterschieden. Betroffen sind insbesondere übergewichtige dunkelhäutige Kinder afrokaribischen Ursprungs. Bei nicht eindeutiger Ätiologie scheint die pathologische Verbiegung sekundär durch eine mechanische Fehlbelastung des medialen Kompartiments mit Verzögerung des Wachstums der medialen Tibiaepiphyse verursacht zu werden. Wichtige Faktoren der mechanischen Überlastung sind das häufig präsente Übergewicht und ein Vitamin-D-Mangel. Klinisch zeigt sich eine variable Ausbildung der Varusdeformität der proximalen Tibia, eine vermehrte tibiale Innenrotation, Gangstörungen sowie in unilateralen Fällen eine Beinlängendifferenz. Typische radiologische Veränderungen werden in diversen Klassifikationen beschrieben. Therapeutisch kann konservativ mittels Orthese nur bei der milden infantilen Form behandelt werden. Sonst ist die Therapie immer operativ, wobei je nach Alter und Ausprägung verschiedene Verfahren zur Anwendung kommen.
