Zusammenfassung
Pseudorheumaknoten (Synonym subkutanes Granuloma anulare) sind v. a. im Kindes- und Jugendalter auftretende, subkutan gelegene granulomatös-entzündliche Läsionen unklarer Ätiologie. Sie manifestieren sich als nicht schmerzhafte, nicht verschiebliche subkutane Knoten mit initial oft rascher Größenzunahme. Prädilektionsstellen sind die Tibiakante, der Fußrücken sowie der Kopf. Die Granulome können solitär oder multipel auftreten, über Monate bis Jahre persistieren und lokal oder entfernt rezidivieren. Klinisch und histopathologisch ähnelt das subkutane Granuloma anulare den Rheumaknoten bei der juvenilen idiopathischen Arthritis, eine Assoziation zu entzündlich-rheumatischen Erkrankungen besteht jedoch nicht. Weitere wichtige Differenzialdiagnosen sind Fremdkörpergranulome, subkutane Hämatome, die Sarkoidose oder infektiöse granulomatöse Erkrankungen. Pseudorheumaknoten weisen eine hohe spontane Rückbildungstendenz auf, weshalb therapeutische Maßnahmen meist nicht erforderlich sind.
