Zusammenfassung
Frontallappenepilepsien machen knapp ein Viertel aller fokalen Epilepsien aus, aufgrund der Mannigfaltigkeit der funktionellen Anatomie dieses größten Hirnlappens können die epileptischen Anfälle sehr vielgestaltig sein. Sie sind oft kurz, schlafgebunden und ohne Einschränkung des Bewusstseins. Typische Anfallssemiologien umfassen uni- und bilaterale klonische und/oder tonische, hyperkinetische, nicht-bewusst erlebte und oral-motorische Zeichen. Die mitunter bizarre Symptomatik kann zur Fehldiagnose von psychogenen nicht-epileptischen Anfällen führen. Durch rasche Propagation epileptischer Aktivität in benachbarte temporale Strukturen kann der Anfallsursprung fälschlicherweise in diese Region projiziert werden. Das iktale EEG ist oft nicht hilfreich, da es von deutlichen Bewegungsartefakten überlagert oder komplett unauffällig sein kann. Bei Pharmakoresistenz sollte die Option einer Resektion des Anfallsfokus geprüft werden, postoperative Anfallsfreiheit kann bei etwa jedem zweiten Patienten erreicht werden.
